In the event that Dr. Jalazo is an out-of-network provider for your insurance company, she will gladly furnish you with an Mental Heath Encounter Form (signed receipt for services) to send to your insurance company for reimbursement.

FREE PSYCHOTHERAPY FOR VETERANS

Dr. Jalazo is a proud volunteer of the Veteran’s Project of South Florida and as such provides free psychotherapy services to our country’s veterans on a limited basis.  If you are a veteran and are in need of assistance, please contact the project’s administrator.
